WHAT:

Celebration of the Five Wishes advance health care directive; announcement of 2011 accomplishments; award presentation.

More than 200 Five Wishes participants, volunteer trainers, and members of the community will be in attendance. The event is open to the public and free for individuals and groups. Reservations are recommended, but drop-ins are welcome.

Event speakers include County Supervisor Salud Carbajal; Mayor Helene Schneider; Ron Werft, CEO of Cottage Hospital; Dr. Michael Bordofsky; Paul Malley, President of national nonprofit Aging with Dignity; Susan Plummer, Executive Director of the Alliance for Living and Dying Well; and guest emcee Catherine Remak.

About the Alliance for Living and Dying Well

The Alliance for Living and Dying Well is a collaborative of Santa Barbara County organizations concerned with quality end of life care. A primary goal of the Alliance is to provide supportive opportunities for community members to have the conversations necessary to complete useful and meaningful Advance Health Care Directives. The Alliance is utilizing the Five Wishes document as a guide for these very important discussions with loved ones and doctors. In addition to letting those close to us know our health care wishes, these conversations can actually empower us to live more fully.

The Alliance provides many opportunities for the community to learn about and complete their Five Wishes, including family and friends events within faith congregations, senior living communities and many other settings. The Alliance also provides free training for those who wish to learn how to lead Five Wishes groups or community events.

The Alliance for Living and Dying Well includes core supporters for and providers of compassionate end-of-life care including: Archstone Foundation, Cottage Health System, Hospice of Santa Barbara, James S. Bower Foundation, Sarah House, St. Francis Foundation, Visiting Nurse & Hospice Care and retirement communities.
