Join us for a special 2-part workshop and complete your MyCare advance directive. Advanced care planning is a way for adults of all ages, in all stages of life to prepare for the future. Advance care planning involves:

  • Making decisions now about the types of health care you would and would not want to receive if you become very sick or injured and cannot speak for yourself
  • Choosing a trusted person to make health care decisions on your behalf if you were unable to do so yourself
  • Talking with your doctors and loved ones about your wishes and healthcare goals
  • Documenting these decisions in a legal document called an advance directive. In Santa Barbara County, we recommend using the MyCare Advance Directive.

Day 1 – Thursday, April 10, 1:00 – 2:30 – Overview of advance care planning, health care decisions, and MyCare

Day 2 – Thursday, April 17, 1:00 – 2:30 – One-on-one help from experts and free notarization (for advance directives only). If you are updating a current advance directive or only need notarization, you have the option of attending Day 2 only.

This program is free and open to the public. To RSVP, please call  (805)-705-0546 or email CommunityACP@hospiceofsb.org. We look forward to seeing you there.
